Please read these terms carefully before enrolling in the My Personal Pharmacy membership program.

1. Program Description
My Personal Pharmacy is a prescription membership program offered by Concierge Medical Marketing LLC in partnership with ReviveHealth. This program provides access to a defined list of included medications for a monthly household membership fee. This program is not insurance and is not regulated as an insurance product.
2. Membership Eligibility
Membership is available to households, including all eligible members residing at the same address. Children up to age 26 who reside elsewhere may also be eligible. All members must comply with program rules and provide accurate enrollment information.
3. Included Medications
Included medications are listed on the Included Medications page and are subject to change without notice. Medication availability, quantity limits, and fulfillment rules apply. A valid prescription from a licensed prescriber is required for all medications. All medications are manufactured in the United States of America.
4. Membership Fees
The current membership fee is $29 per month per household, subject to change. Fees are billed on a recurring basis. Cancellation policies and refund terms may apply and are disclosed at enrollment.
5. No Insurance Coverage
This program is not insurance. It does not provide coverage for medical services, hospitalization, or medications not included on the program list. Members are responsible for obtaining separate health insurance if desired.
6. Limitation of Liability
Concierge Medical Marketing LLC and ReviveHealth are not responsible for any adverse medical outcomes, side effects, or health issues resulting from the use of medications obtained through this program. Members are solely responsible for discussing their medications with their licensed healthcare provider and understanding the risks and benefits of each medication prescribed.
7. Program Changes and Termination
The program, included medication list, pricing, and rules may be modified, suspended, or terminated at any time. Members will be notified of material changes in accordance with applicable laws.
8. Governing Law
These terms are governed by the laws of the state in which Concierge Medical Marketing LLC is organized. Any disputes shall be resolved in accordance with applicable law.
